Varenicline (Champix)

Varenicline PGD Service for Stoke
To provide the service the pharmacy must have signed up to the SLA and the pharmacist MUST attend the training provided by Stoke on Trent City Council.

Consultations are recorded on PharmOutcomes and you will only have access to the module when your pharmacy is signed up to the SLA and the pharmacist is fully trained.
Where no live connection to PharmOutcomes is available you can use the Proforma but must add the information to PharmOutcomes as soon as possible and always within 48 hours of your consultation with the patient.

Supplies beyond 12 weeks
In some special circumstances the Public Health Commissioner may authorise a patient to continue on Varenicline for up to a further 12 weeks. The supplying pharmacy needs to email Rochelle Edwards on Rochelle.Edwards@stoke.gov.uk to request authorisation. If this is approved you must use the ProForma below and record supplies on the special week 13 to 24 PharmOutcomes module.
